Revolutionising shared patient care

eSP (electronic Shared Plans) enables the sharing of care plans between all carers responsible for a patient. Using the simple and clear interface, GPs or HCPs in other places of care can access the latest plan for each patient with the ability to easily update or create a new plan pre-populated with information from the patient’s GP record. On creation or amendment of a plan, all updates can be saved back to the GP patient record so the completeness and integrity of the GP patient record is maintained at all times.

 

End of Life

eSP can be used to underpin a range of care plans and is currently being used to power an Electronic Palliative Care Co-ordination System (EPaCCS). This records patients’ preferences as they near the end of their life, automatically sharing information with the ambulance service, Out of Hours and 111. Currently available for EMIS Web practices, the solution is rolling out in Worcestershire, Herefordshire, Coventry and Warwickshire with integration to Adastra available in the first quarter of 2016.
